United States Shorter Lives, Poorer Health report model 

In 2013, a joint paper from the US National Research Council and the Institute of Medicine prepared a large report comparing health indicators for the US with those in 16 other developed countries. It gives a devastating review of how the US ranks, and is interesting reading.

The report discusses determinants, and bases their analysis on the social ecological framework presented below:
