As a clinician you will deal mainly with individual patients whose health problems are due, in part, to environmental and behavioral factors. These, in turn, are caused by broader social forces that are commonly called 'determinants': the causes of the causes. While you may wish to help the patient alter his or her lifestyle to improve their health, success may be limited if they continue to live in the situation that gave rise to those lifestyles. Hence, you should be aware of the broader forces in society that ultimately determine health.
